Step 1
You are invited
Every health journey is unique. One of our care advocates—with a proven record of helping people get well—will call you.
Step 2
Join the program
When you participate in Care Navigation, you’ll be assigned a care advocate, who is your go-to from the moment you agree to join until you graduate the program. Together, you decide on a care plan, which serves as the roadmap for making sure you get better, stay out of or get discharged from the hospital, and make smart long-term decisions.
Step 3
Advocate every step
While you focus on getting better your care advocate is focused on other things, like navigating the healthcare system for you, working with your broader care team, keeping you informed, doing a home assessment when appropriate and helping you clear any hurdle that gets in your way.
Step 4
Graduate the program
The goal of the program is to get you healthy and on a path to recovery in three months. Sometimes it will take longer. In these cases, you’ll have your dedicated care advocate longer to ensure you are recovering.
