BlueAdvantage Administrators groups must purchase Health Education Programs for their members. Check with your Group Administrator or Customer Service Representative to see if you have this benefit before completing an enrollment form.

	You also can call your nearest office location and ask for a Registered Nurse case manager to discuss the program and receive assistance in enrolling in the Diabetes Education Program.
